What We Offer
Health Supervision and Nursing Care
- A full-time registered nurse (RN) is always on duty.
- A licensed practical nurse (LPN) is always on duty.
- Our medical director (MD) ensures that participants’ private physicians’ orders are strictly followed.
Social Services for Participants and Families
- Initial interviews, referrals, home visits, and counseling.
- Guidance through the complexities of Medicare and Medicaid regulations.
- Connection to community services that assist with various issues.
Recreational Activities
- Daily exercise, games, crafts, and movies.
- Educational programs, guest speakers, and entertainers.
- Day trips organized by New Horizons Behavioral Health.
Nutritious Meals
- All meals are tailored to individual dietary needs.
- We offer nutritious breakfasts, hot lunches, and take-home snacks.
Coordination and Referral Services
- Coordination and referral for services with licensed providers as needed, including:
- Dental

FREE Transportation
- Transportation to and from physician appointments.
